Here’s a worksheet that worked well when I introduced my art students to different ways to create a range of value in drawings, as well as modeling form starting with flat shapes.
 
I have expanded my original worksheet design from two to four pages and added samples.

1-Page Worksheet (PDF)
A simplified worksheet with just the shapes to shade in. To save on ink and paper, you could laminate the 4-pager for re-use, and print only this one-sider for student completion.
